CHANGES OF SERUM RBP, SERUM CYSC, AND URINE MALB LEVELS IN PATIENTS WITH CHRONIC GLOMERULONEPHRITIS AND THE PROGNOSTIC VALUE THEREOF

Objective: To analyze the changes of serum retinol-binding protein (RBP), serum cystatin C (Cys C), and urinary microalbumin (mALB) levels in patients with chronic glomerulonephritis and the prognostic value of these indicators. Method: A total of 65 patients with chronic glomerulonephritis, admitted to our hospital from May 2017 to January 2019, were selected as the nephritis group. Another 50 healthy people who came to our hospital for physical examination were selected as the control group. The levels of renal functional indicators (blood urea nitrogen [BUN], serum creatinine [Scr], and serum uric acid [UA]) in the two groups were compared; the changes of serum RBP, Cys C, and urine mALB levels in the two groups were detected and compared by immunoturbidimetry; ROC curve was used to test serum RBP, Cys C, and urinary mALB to assess their prognostic value for patients. Results: The renal function indexes of BUN, Scr, and UA in the nephritis group were significantly higher than those in the control group, and the difference was statistically significant (P<0.05). The levels of serum RBP, Cys C, and urine mALB in the nephritis group were significantly higher than those in the control group, and the difference was statistically significant (P<0.01). The levels of serum RBP, Cys C, and urinary mALB in patients with renal failure were significantly higher than those with remission or no progression; the difference was statistically significant (P<0.01). It can be seen that serum RBP, Cys C, and urinary mALB levels were positively proportional to the prognosis in patients. The higher the level of the three indicators, the worse the patient's prognosis. ROC curve analysis showed that the areas under the curve of serum RBP, Cys C, and urine mALB were 0.801, 0.789, and 0.698, respectively. The areas under the combined index of the three indicators was 0.898, the sensitivity, was 84.4%, and the specificity was 88.9%, and both higher than three separate tests. Conclusion: In the nephritis group, the levels of RBP, Cys C, and urine mALB in serum were increased obviously. The combined test of three indicators is of great value in arriving at the prognosis of patients with chronic glomerulonephritis, and is worthy of clinical promotion.
