EFFECTS OF LOSARTAN ON IMMUNE FUNCTION, T CELL SUBSETS AND MICRO-INFLAMMATORY STATUS IN PATIENTS WITH CHRONIC RENAL INSUFFICIENCY

Objective: To investigate the effects of losartan on immune function, T cell subsets and micro-inflammatory status in patients with chronic renal insufficiency (CRF). Methods: One hundred and eight patients with CRF received treatment in the Department of Endocrinology and Nephrology in our hospital from June 2016 to February 2018, and were randomly divided into a study group (n = 54) and a control group (n = 54). The patients in the control group were given the basic treatment of controlling blood pressure, anti-infection, correcting acid-base balance and electrolyte disorder, as well as having a low salt, low fat and low protein diet. The patients in the observation group were treated with losartan on the basis of the treatment of the patients in the control group. Both groups were treated continuously for eight weeks. The immune function [immunoglobulin A (IgA), immunoglobulin M (IgM), immunoglobulin G (IgG)], T cell subsets (CD3+, CD8+, CD4+, CD4+/CD8+), micro-inflammatory status [tumour necrosis factor alpha (TNF-alpha), C reactive protein (CRP) and Interleukin 6 (IL-6)], renal function (RRF) [haemoglobin (Hb), creatinine clearance rate (Ccr), Urea nitrogen (BUN), and serum creatinine (Scr)] levels were observed before and after treatment. Results: After treatment, the level of IgA, IgM, IgG in the study group was significantly higher than that before treatment; however, there was no significant difference between the control group and the study group (P > 0.05). Compared with before treatment, the levels of CD4+, CD3+,CD4+/CD8+ in the two groups were significantly higher than those before treatment, while the levels of CD8+ in the study group were better than those in the control group (P<0.05). After treatment, the levels of TNF-alpha and CRP, as well as IL-6 in the study group and the control group were significantly lower than those before treatment, while the indexes in the study group were significantly lower than those in the control group (P<0.05). After treatment, Hb and Ccr levels of patients in the two groups were significantly increased compared with before treatment, as BUN and Scr levels were significantly reduced; Hb and Ccr in the research group was significantly higher than the control group, while BUN and Scr levels were significantly lower than the control group (P<0.05). Conclusion: Losartan is effective in the treatment of CRF and can effectively improve the immune function and micro-inflammation of patients.
