Renal hyperparathyroidism usually occurs in chronic renal failure dialysis. However, rental hyperparathyroidism resulting from the intrathyroid gland is uncommon. Here, We present the case of a 35-year-old man who had been on haemodialysis for 8 years. He had renal hyperparathyroidism with bone pain for 1 year. The ectopic intrathyroidal parathyroid gland and two others were localised by preoperative neck ultrasonograplay and technetium-99m sestamibi scans with single-photon emission computed tomography imaging. A right thyroid lobectomy was performed and one intrathyroidal parathyroid gland was found, which was excised. The patient recovered uneventfully and his symptoms resolved. Therefore, clinicians should be aware of the possibility of renal hyperparathyroidism resulting from the intrathyroidal parathyroid gland in cases where renal hyperparathyroidism persists after parathyroidectony.
