•Ectopic ACTH producing tumors (EA) can present high morbidity due to hypercortisolism.•EA of unknown (EU) and known origin (EK) have similar demographic and laboratory patterns.•Bilateral adrenalectomy (BA) can be used to cure hypercortisolism in patients with EU and EK.•Survival rates after BA are similar between EK and EU.
