•Stage I mucinous ovarian cancer can be divided into high- and low-risk prognostic groups based on patient characteristics.•The low-risk group of patients with stage I mucinous ovarian cancer may not benefit from chemotherapy.•The high-risk group of patients with stage I mucinous ovarian cancer may benefit from chemotherapy.
