•What is the primary question addressed by this study? The study investigated whether cortical thickness of salience network structures predicted change in apathy following 12-weeks of antidepressant treatment with escitalopram.•What is the main finding of this study? Greater thickness of the right insula at baseline predicted reduction in apathy symptoms following antidepressant treatment. Thus, patients with thinner insula were more likely to exhibit residual apathy symptoms at treatment end.•What is the meaning of the finding? Depressed older adults with thinner insula may benefit from alternative treatment approaches or augmentative interventions that target abnormalities of the salience network.
