•Most patients receiving definitive SBRT for early NSCLC are staged with PET alone.•In patients cN0 by PET, the added benefit of invasive staging (INS) is unknown.•Of 651 patients staged with PET before SBRT, 15.2% received INS.•Addition of INS did not alter patterns of failure or survival after definitive SBRT.
