•SEEG recordings revealed independent bilateral seizures in two thirds of subjects with postencephalitic epilepsy.•Postencephalitic epilepsy was associated with ictal patterns in anteromesial temporal and perisylvian regions.•Perisylvian and anteromesial-perisylvian ictal onsets were unique to postencephalitic epilepsy.
