•The risk of recurrence and treatment options are unsolved issues in resected NSCLC.•Multidisciplinary approaches may implement the actual pTNM-based prognostic score.•Radiomics was exploited to non-invasively decipher tumor immune microenvironment.•We identified a radio-immune signature with prognostic impact in early stage NSCLC.•Integrating imaging and tissue features may enclose new risk stratification models.
