•Anomalies of the inferior vena cava can cause unprovoked deep venous thrombosis.•Thrombolysis is a safe treatment option with good functional outcome in patients with iliofemoral deep venous thromboses in absent inferior vena cava.•Thrombolysis treatment may reduce the incidence of post-thrombotic syndrome.
