•Rectal GIST is a rare entity of soft tissue sarcoma•Most rectal GISTs are resected surgically and treated with perioperative imatinib•Despite multimodal treatment, the recurrence rate is high•Aspects of care are of higher quality in specialist centres•Referral of rectal GIST to specialist centres is therefore recommended
