•A highly sensitive UPLC-MS/MS method was developed for the quantification of paromomycin in human plasma.•Linear calibration model with a concentration range from 5 to 1000 ng/mL.•Easy protein precipitation sample preparation using trichloroacetic acid.•Ion-pair chromatography provides sufficient selectivity between paromomycin and the biological matrix.•Polypropylene containers and acidic conditions prevented paromomycin adsorption.
