•Non-vitamin K oral anticoagulants (NOAC) during pacemaker or defibrillator surgery are safe.•Either NOAC continuation or interruption appears reasonable for atrial fibrillation.•Incidence of bleeding and thromboembolism is low.•Incidence of pocket haematoma appears numerically higher with NOAC continuation.
