•Almost 40% of the DDIs were perceived as not clinically relevant for the ICU setting.•In the group of frequently occurring DDIs, 63% of the DDIs were perceived as not clinically relevant.•The number of DDI alerts shown to ICU prescribers can be substantially decreased.
