•Can a caregiver-directed skills-based intervention prevent (the development of) aggressive behavior in individuals with dementia?•The caregiver-directed skills-based intervention did not reduce (the development of) aggressive behavior over time compared to a group that received usual care and printed resource materials.•Given the gravity of aggressive behavior and the modest efficacy of available pharmacologic and nonpharmacologic interventions, novel preventive approaches are urgently needed.
