We recently encountered an 87-year-old male patient, who presented with recent onset of lumbar radiculopathy. Over seven years prior to the radiculopathy, he was found to have a 3.2 cm renal mass that was radiologically stable on initial active surveillance. Imaging studies at the time of presentation revealed progressive increase of the right renal mass (to 5.6 cm) as well as an additional 3.4 cm destructive mass involving the right posterior L2 vertebral body.
