A study of 105 cases with acute cholecystitis and cholangitis using multichannel gene autoanalyzer

Introduction: Acute cholecystitis and cholangitis are inflammatory diseases that can be fatal if treated by inappropriate antimicrobial regimen. Presently, it takes approximately 4-5 days to obtain bile culture drug susceptibility results, and an inappropriate antimicrobial treatment during the period, before the results, hinders rapid treatment. We evaluated the utility of multichannel gene autoanalyzer using the bile specimens of acute cholecystitis and cholangitis to resolve this issue. Method: We studied 105 patients diagnosed as acute cholecystitis and cholangitis from 2015 to 2018 at our hospital. Nosocomial culture test and analysis using multichannel gene autoanalyzer were performed using bile samples collected from all patients, and the results were comparatively analyzed. In this study, the Verigene® system was used as a multichannel gene autoanalyzer. Result: Of the 105 patients, 13 were positive in the Verigene® system of the 55 patients positive for the bile culture. Of the 50 patients showing negative bile culture result, all patients were negative in the Verigene system. Four cases of Extended-spectrum β lactamase(ESBL) were detected in the drug susceptibility test, three of which were detectable in the Verigene® system. Conclusion: The Verigene® system was found to be useful as a screening test for drug-resistant bacteria, such as ESBL, in the rapid identification of bile bacteria in patients with acute cholecystitis and cholangitis.
