Risk Factors for Pulmonary Complications After Hepatic Resection : A Retrospective Observational Study

Introduction: Postoperative pulmonary complications (PPC) after hepatic resection are the commonest complications and associated with increase the length of hospital stays. Although several studies have focused on the risk factors for PPC after abdominal surgery, few have investigated the risk factors for PPC after hepatic resection. The purpose of this study was to identify the risk factors for PPC after hepatic resection. Method: This was a retrospective study. Data on 368 patients who underwent hepatic resection between January 2010 and December 2018, were retrieved from a database. We retrospectively evaluated the incidence of PPCs and their risk factors in hepatic resection. Result: In 368 patients who underwent hepatic resection, the incidence of PPC was 37.5% (n=138) including symptomatic pleural effusion in 27.7% (n=102), pulmonary infection in 17.4% (n=64), pulmonary embolism in 7.1% (n=26), and acute respiratory distress syndrome in 6.8% (n=25). Multivariated logistic repression analysis revealed that the risk factors for PPC were open surgery (HR: 2.382, 95% CI: 1.162-7.014: P=0.028), posteosuperior resection (HR: 2.134, 95% CI: 1.454-3.875: P<0.001), transfusion (HR: 2.752, 95% CI: 1.957-3.985: P=0.038). The length of postoperative hospital stay (14.4 ± 5.8 days vs. 8.3 ± 5.7 days, P = .001) were significantly longer in the patients with PPC. Conclusion: We founded risk factors after hepatic resection. Our date proved useful information to prevent of PPC and reduce the length of hospital stays in patients underwent hepatic resection.
