•A total of 1355 patients who received primary PCI due to STEMI were analyzed in this study•D-dimer level was an independent predictor of in-hospital MACE•In the subgroup of AET and no-reflow phenomenon, D-dimer remained as an independent risk factor of MACE•D-dimer was not associated to no-reflow phenomenon in STEMI patients
