•The impact of elevated/irritable mood and increased activity/ energy as equal and necessary criterion A symptoms for a diagnosis of (hypo)mania in DSM-5 is poorly elucidated.•Patients with bipolar disorder evaluated mood, irritability and activity level daily during follow-up via a smartphone-based system.•Co-occurring elevated/irritable mood and activity was prevalent 0.12% of the time compared to 24% with the time with elevated/irritable mood only.•Based on real life daily assessments, the prevalence of (hypo)manic episodes is substantial reduced as a result of the introduction of DSM-5 and with potentially clinical consequences.
