•Endothelial injury and platelet activation are common in patients resuscitated from cardiac arrest.•Therapeutic interventions towards the endothelium may improve outcome.•Low dose Iloprost (1 ng/kg/min) did not influence platelet aggregation.•Low dose iloprost did not increase risk of bleeding in out-of -hospital cardiac patients.
