What are the differences between moderate and severe COPD exacerbation groups

Aim: This study aimed to investigate the relation between exacerbation severity and quality of life, dyspnea level, exacerbation frequency in the past and in the future. Methods: The patients that treated with SABDs plus antibiotics and/or oral corticosteroids were considered moderate;the hospitalized ones were considered to have severe exacerbation. Dyspnea level, pulmonary functions, number of exacerbations and quality of life scores were compared by exacerbation severity. Results: A total of 103 patients with exacerbation were enrolled in this prospective study between May-December 2018. There were 38 moderate, and 65 severe exacerbations. The mean number of exacerbations in the last year was 2.45±2.06. The number of exacerbations (p<0.001), the number of severe exacerbations in the last year (p<0.001), and during follow up (p=0.003) were greater in the severe exacerbation group. The severe exacerbation group had a higher CAT (p=0.013), and mMRC (p<0.001) scores, and a lower FEV1 (p<0.001). The severe exacerbation group also had lower scores of SF-36 domains (Physical functioning (p=0.001), role limitation (p<0.001), emotional well-being (p<0.001), social functions (p<0.001), general health (p=0.002), and energy (p=0.002). Conclusion: Patients with severe exacerbations had an accelerated decline of lung function, a worse quality of life, and an increased risk of experiencing frequent exacerbations.
