•PNH is rare in patients undergoing CEA during the first 14 days of symptom onset.•PNH is not related to increased risk of cardiovascular or poor functional outcomes.•Treatment with therapeutic anticoagulation might be associated with a higher risk of PNH.•The role of anticoagulants in PNH after CEA needs further evaluation.
