P19.11: Nomogram for prediction of preterm birth in symptomatic uterine contraction in twin pregnant women

Preterm labour is one of the most common complications in multifetal pregnancies but optimal management of preterm labour in multifetal pregnancies has not been established. Accurate prediction of impending preterm birth is important issue in clinical decision for administration of antenatal corticosteroid or tocolytics and transfer to referral hospitals. The objective of this study was to determine clinical factors which can predict the risk factor and to develop nomogram for prediction of preterm birth (PTB). In prospective cohort, twin pregnant women who had symptomatic uterine contractions at < 34 weeks of gestation without evidence of ruptured membrane were included. Clinical factor were compared according to occurrence of PTB within 7 days. By multivariate analyses and logistic regression, we developed prediction model for PTB within 7 days. During study period, a total of 113 twin pregnant women were included. Women who delivered within 7 days had more frequent uterine contractions, higher NRS pain score, more advanced cervical dilatation, shorter cervical length (CL) and higher rate of positive fibronectin (FBN) test. In logistic regression analysis, NRS pain score, CL and positive FBN were included in prediction model and the area under the ROC was 0.872 by three factors. In addition, we developed static nomogram for prediction of PTB within 7 days.
