Stress cardiomyopathy could complicate severe traumatic brain injury and subarachnoid hemorrhage, and potentially lead to myocardial dysfunction, acute heart failure and death. Prompt recognition and timely eCPR can provide effective rescue. A 12 year old male was airlifted from the beach after he was struck in the back of his head with a “bocce ball”. On arrival, he was hypotensive, had copious frothy bloody tracheal secretions.
