•Tidal expiratory flow limitation promotes dynamic hyperinflation in COPD.•Dynamic hyperinflation increases ventilator asynchronies and work of breathing.•Monitoring expiratory flow limitation allows real-time PEEP tailoring to abolish it.•PEEP selected by this new method were variable and different to those prescribed.•Optimal PEEP increased from seated to supine position by patient-specific amounts.
