•NAT + DR assay was developed for the combined detection of ART failure and resistance.•The NAT + DR method was highly cost-effective compared to conventional methods.•The assay characteristics of NAT + DR assay was 100% compared to conventional testing.•The NAT + DR assay was highly congruent with the conventional method of testing.
