•Chronic ambulatory baseline ECoGs captured over 7 years from mesial temporal lobe (MTL) epilepsy patients were analyzed.•Interictal spike rate strongly differentiates MTL patients with best clinical outcomes.•Baseline ECoGs captured during months with no clinical seizures had the least interictal spike rate.
