•The overall concordance of CMV DNA detection in neonatal saliva and urine was 91%.•Compared to urine, positive predictive value of CMV PCR in neonatal saliva was low.•Viral load in saliva was significantly lower in false-positive than in true-positive samples.•For cCMV diagnosis after maternal primary infection urine is the preferred sample.
