We congratulate Cronin et al for the significant reduction in the number of blood transfusions that they achieved by implementing a perioperative surgical home (PSH) strategy during spinal fusion procedures . However, as the authors themselves admit, the exact causes for this well achieved target remain unclear, as the PSH is a multi-faceted strategy. Clearly, the implementation of various blood-conservation strategies even in the absence of a low pre-defined haemoglobin (Hb) value as a transfusion threshold is expected to lead to fewer blood transfusions.
