Abstract Herein, we describe a case of failure of delayed breast reconstruction due to leech-borne infection by extended-spectrum β-lactamase (ESBL)-producing and extensively-resistant A. hydrophila. In the current era of increasing antibiotic resistance, such infections should be considered to redefine preventive protocols of leeches safety and search of bacterial resistance to antibiotics.
