Asymmetric distributed multiple cysts in basal ganglia and corona radiata in two patients

•Extreme-enlarged VR space is rare and benign with specific imaging characteristics.•Mostly no specific clinical manifestations and intervention were needed.•Differentiation of extreme-enlarged VRS from other pathologic process is important.
