•Percutaneous coronary intervention in patients with diabetes mellitus remains associated with inferior clinical outcomes with non-diabetic patients even in the era of drug-eluting stents (DES).•We performed a meta-analysis of all published studies reporting the outcomes with drug-coated balloons (DCBs) vs. DES after de-novo coronary PCI in diabetic patients.•DCBs are associated with similar clinical and angiographic outcomes compared with first-generation DES in diabetic patients,•DCBs might be associated with a potential reduction in TLR, but this needs a large RCT powered to identify that benefit
