•We assessed the utility of peripheral NLR as a predictor of outcomes in preterm infants with IVH.•Rates of seizures in this cohort correlated with higher NLR (>3).•Neonatal seizures increase morbidity in the preterm infant population.•The NLR is a simple and cost-effective measurement of inflammation.
