•There is a paucity in the literature of studies that have evaluated endotracheal tube (ETT) intracuff pressure in ventilated patients during helicopter emergency medical service (HEMS) transport.•Two prospective observational studies suggest that ETT intracuff pressure exceeds the critical perfusion pressure of the trachea during HEMS transport.•Potential clinical implications range from minor to major complications.•The true incidence of these problems after transient increased cuff pressure is unknown.•Further research is required to determine if these findings are truly significant with longitudinal follow-up to determine if the effects of transient over- and underinflated ETT cuffs after HEMS transport are clinically significant.
