Donor-derived Strongyloides stercoralis Infection in a Kidney/Pancreas Recipient, 2014: Evidence of the Need for Targeted Donor Screening

Index Case The index case was a 36 year old US-born male with history of diabetes. He received a left kidney and pancreas transplant. Five weeks post-transplant he developed nausea, abdominal pain and diarrhea. He was admitted to the hospital seven weeks post-transplant. Duodenal u0026 skin biopsy showed Strongyloides larvae. He was treated with ivermectin and albendazole and his immunosuppression was decreased. He developed Klebsiella bacteremia and both organs were removed due to concern for rejection. Pre-transplant serum was NEGATIVE for Strongyloides antibody.
