•About 53.53% of patients with drug-resistant epilepsy achieved >50% reduction of seizure frequency after VNS.•EEG feature and heart rhythm complexity may be predictors of response to VNS therapy.•Drug-resistant epilepsy patients with shorter duration of epilepsy may be better candidates for VNS.
