•The safety of allopurinol in pregnancy has been largely debated.•A total of 53 allopurinol exposed infants has been described in the literature.•Allopurinol teratogenicity has been reported in two cases with uncertain causality.•Report of all exposed infants (deviant and normal cases) should be encouraged.
