•Patients with MAC are typically elderly patients with prohibitive surgical risk.•In high-risk patients transcatheter Valve-in-MAC may be considered.•Iatrogenic ASD closure in cases of thrombus is essential to minimize embolization.•Careful patient selection is fundamental to reducing complications.
