Middle Cerebral Artery Territory Infarct Secondary to Carotid Artery Dissection: A Case Report and Brief Review

We report the case of a 42 year-old man with a right middle cerebral artery territory infarct secondary to dissection of the right internal carotid artery who was followed throughout presentation, acute management and recovery. Cervical artery dissection (CAD) is one the most common causes of ischemic stroke in young and middle aged patients. Furthermore, spontaneous dissection of the carotid and vertebral arteries have a largely unknown pathogenesis. This case highlights how CAD may complicate treatment decisions concerning the effectiveness of thrombolytic therapy, the role of endovascular treatment, optimal antithrombotic therapy, and prevention of recurrence.
