Retrospective analysis of irrational prescription in pharmacy intravenous admixture services

Objective To analyze the irrational prescription of pharmacy intravenous admixture services(PIVAS)in our hospital,and to provide reference and a basis for pharmacists and clinical rational drug use respectively.Methods The prescription of pharmacy intravenous admixture services from January to June in 2013 were statistically classified and analyzed according toNew Sexual Pharmacy(the 17th edition),The Application Guide of Injectable Drugs,The Safety Application and Compatibility for 400 Kinds of Injection,Practical Manual in PIVASand Package inserts of drug.Results There were 115 irrational prescriptions(accounted for 0.075%)in 154269 prescriptions.The irrational prescription were found,including drug concentration,dose,solvent,compatibility,none reference and computer entry errors.Conclusion The pharmacist plays an important role in rational drug use.Through the prescription intervention of pharmacist,the drug safety was insured and the communication with clinicians and nurses and pharmacists were enhanced.
