•Urinary 8-hydroxy-2′-deoxyguanosine (U-8-OHdG) is a biomarker of oxidative stress.•U-8-OHdG levels were higher in active cardiac sarcoidosis (CS) than in non-active CS.•U-8-OHdG was an independent predictor for cardiovascular-related death in active CS patients.•CS patients with elevated U-8-OHdG levels might be resistant to corticosteroid therapy.
