Setup uncertainties in linear accelerator based stereotactic radiosurgery and a derivation of the corresponding setup margin for treatment planning

•Image guidance is critical to frameless stereotactic radiosurgery for accurate setup.•A correlation is postulated to exist between setup margin and setup uncertainties.•Minimal geometric margin might be smaller than the corresponding 3D setup error.•Formulas are proposed to calculate setup margin based on institutional clinical data.
