•There is little research analyzing the economic impact of SRS vs Whole Brain Radiotherapy (WBRT) for brain metastases.•The initial decision to perform upfront SRS is a significant economic decision.•Using WBRT decreases the cost of care, though with increased toxicity.•A high brain metastasis velocity (BMV) increases the cost of care per unit time.
