•Philadelphia chromosome–like (Ph-like) acute lymphoblastic leukemia (ALL) is not uncommon in adults and is associated with poor outcomes with conventional therapy.•The role of consolidation with allogeneic hematopoietic cell transplantation in adults with Ph-like ALL in first complete remission remains unknown, and thus there is an urgent need for developing well-designed studies to address this important question.•Novel therapies including immunotherapy and kinase inhibitor therapy in the peritransplantation period may be options to improve outcome of Ph-like ALL.
