The epilepsy transition care gap in young adults with childhood-onset epilepsy.

•Only one-third of young adults who had active epilepsy prior to age 18 had epilepsy transition preparation discussions.•Transition discussions were associated with a history of a comorbid neurodevelopmental disorder for patients with active epilepsy.•Less than one-half of cases with active epilepsy at the time of transition now report being cared for by an adult epilepsy provider.
