•HCT donor inflammation has not been investigated as a risk factor for GVHD.•Donor serum IL-1β and grades III to IV acute GVHD were associated in a test cohort.•Donor serum TNF-α and chronic GVHD were inversely associated as well.•Neither finding was validated in a replication cohort.•Transferability of donor characteristics remains a key area for future research.
