Effectiveness of increasing the frequency of posaconazole syrup administration to achieve optimal plasma concentrations in patients with haematological malignancy

•Increasing the frequency of posaconazole syrup is effective for adjusting drug levels.•Optimal drug levels are not achieved when steady-state level is <200 ng/mL.•In only 5.3% (6/114), drug levels remained suboptimal despite dose adjustment.
