•The relationship between proteinuria and cognitive dysfunction in LBD was assessed.•The urine P/C ratio was independently associated with decreased cognition in LBD.•Our findings suggest that subclinical renal vascular injury affects cognition in LBD.•Evaluation of proteinuria might be helpful for assessing vascular injury in LBD.
