•There are very few reports on surgical treatment of odontoid fracture in patients with long-standing AS.•CT is the best investigation to demonstrate occult odontoid fractures in patients with cervical kyphosis owing to AS.•Posterior occipitocervical fusion is effective for patients with AS.
